VA Form 21-526EZ — Application for Disability Compensation

What it is VA Form 21-526EZ is the Application for Disability Compensation and Related Compensation Benefits. It is the form VA uses to decide service connection, assign a rating, and start monthly compensation. The current revision is dated January 2026.

When you use it Use it to file an original claim, add new conditions, request an increased rating on a condition you already have, or apply for Individual Unemployability (TDIU). If you already submitted an intent to file (VA Form 21-0966), submit the 526EZ within one year to lock in the earlier effective date.

What to gather before you fill it out DD-214 or other separation documents, a list of every condition you are claiming with the body system affected, VA and private treatment records tied to those conditions, dates and locations of relevant in-service events, current address and direct-deposit info, and the names and contact info of any private providers you want VA to request records from.

You can also mail the paper form to the Claims Intake Center, PO Box 4444, Janesville, WI 53547-4444, fax it, hand-deliver it to a VA regional office, or file through an accredited representative.

Common pitfalls Leaving claimed conditions vague (write the specific diagnosis or symptom, not just "back"). Forgetting to list every private provider, which forces you to chase records later. Skipping the certification signature page on the paper form — an unsigned 526EZ is not a complete claim under 38 CFR 3.160.
